Can I customize the portfolio category page to show a grid view of the portfolio categories instead of all the individual portfolio entries?
I'd like to apply the same grid styling, but only showing the portfolio categories. Each one would then link to a custom category page showing the portfolio entries, like this: http://dev.stsllc.net/representative-engagements/healthcare/
The category pages inherit default post layout.
In order to get have grid layout you'd need to customize the theme code.
Although, the grid layout does have option of filtering the articles by category, by setting 'Category Filter' field to 'yes'.
That way, when you select one category, it will list only posts from that category, and the link is to the single post.

Actually, I could hard code the category selection page I suppose. My client likes the category filter, but he wants to force people to choose a category first - he doesn't want all the portfolio pieces showing up. So basically I need to make up a page with a grid, but each box links to the category page. I suppose I could just copy the CSS for the boxes and hard-code them into the page. Would that work?
Should you just need a page to link to separate categories, you can create it through Bold Builder, by simply adding the title of each category, and, for example, image, that will have the link to the page that lists posts from that category.
But, unfortunately, like we noted, the default category pages (with the list of posts), cannot be edited, unless you hard code them by editing the specific files.

- At the bottom of each Portfolio post page template is a next and back button for the next and previous portfolio post. How do I remove this?
- The default URL for Portfolio pieces is "portfolio". Would I be able to change this? so instead of "/portfolio/assessment-reduction" I would have "/representative-engagements/assessment-reduction".
In order to remove next/previous buttons, please add the code below to Custom CSS field in Appearance >Customize >General Settings:
In order to change portfolio slug, please go to Plugins >Editor, on your dashboard, and select finance.php file. In that file, locate register_post_type function. Below it, you'll find the following line of code, which you should edit:
